About the Role


Make sure you are working with the PSS as a team coordinator for your caseload clients.
Provide PSS guidance and resources for the clients.
QP: participate in diagnosis and treatment planning.
Coordinating treatment
Providing proactive follow-up of treatment response
Updating the PCP when the patient is not improving.
Support and document the medication as part of medication management.
Make sure all consumers are getting annual check-ups (Physicals, dental and eye exams)
Facilitating communication with the clinical consultant regarding treatment changes
Offer brief counseling (using evidence-based techniques such as motivational interviewing, behavioral activation, and problem-solving treatment)
Provide full wrap-around services (Linking ALL providers together with the consumer to create a person-center plan that is individualized and benefits the consumers)

Requirements

  • A Bachelors Degree

  • Demonstrated ability to collaborate and communicate effectively in a team setting.

  •  Ability to maintain effective and professional relationships with patient and other members of the care team.

  •  Experience with screening for common mental health and/or substance abuse disorders.

  • Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders, when appropriate.

  • Ability to effectively engage patients in a therapeutic relationship, when appropriate.

  • Ability to work with patients by telephone as well as in person.

  • Experience with assessment and treatment planning for common mental health and/or substance use disorders.

  • Working knowledge of evidence-based psychosocial treatments and brief behavioural interventions for common mental health disorders, when appropriate (e.g., motivational

  • Interviewing, problem-solving treatment, behavioral activation).

  • Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of practice for type of provider filling role.

  • Experience with evidence-based counseling techniques
