About the Role

Client Support and Assistance:

Provide direct support to individuals in the community who may be facing challenges such as mental health issues, addiction, homelessness, disabilities, or family struggles.
Assist clients in setting goals for their personal development and recovery, such as improving life skills, achieving employment, or gaining independence.
Case Management:

Develop individualized support plans that outline client goals and needed resources.
Monitor clients' progress toward achieving these goals and adjust support plans as necessary.
Coordinate with other service providers, such as healthcare professionals, housing agencies, or social services, to ensure comprehensive care.
Resource Referral:

Connect clients to community resources and services such as housing, financial assistance, healthcare, addiction treatment, or employment opportunities.
Provide information on available community services and help clients navigate complex systems, ensuring they are aware of their rights and entitlements.
Advocacy:

Advocate on behalf of clients in various settings, including with healthcare providers, social services, and other agencies.
Empower clients by helping them develop self-advocacy skills to manage their own care and make informed decisions.
Crisis Support:

Offer immediate support in crisis situations (e.g., mental health crises, housing instability, or emergencies).
Help clients develop coping strategies and safety plans in times of distress.
Education and Skill Development:

Provide clients with education and support in developing life skills, such as budgeting, time management, job readiness, or parenting.
Facilitate workshops or training sessions to enhance clients' skills and foster independence.
Documentation and Reporting:

Maintain accurate and up-to-date client records, including progress notes, case plans, and communication with other service providers.
Ensure confidentiality and adherence to legal and ethical standards in all documentation.
Community Outreach:

Engage with local communities to identify needs and provide information about available resources.
Build relationships with local organizations, agencies, and community members to strengthen community support networks.

Requirements

  • Education:

    • A high school diploma is typically the minimum educational requirement, though a bachelor’s degree in social work, psychology, sociology, or a related field is preferred.

  • Experience:

    • Previous experience working in social services, mental health, or community-based settings is typically required.

    • Experience in case management or direct support roles is highly valued.

  • Skills:

    • Strong communication skills: Ability to effectively communicate with clients, families, and service providers.

    • Problem-solving and critical thinking skills: Ability to assess client needs and find appropriate solutions or resources.

    • Empathy and active listening: Ability to connect with clients and understand their perspectives.

    • Cultural competency: Ability to work with clients from diverse backgrounds and adapt approaches to their unique needs.

    • Organization and time management: Ability to manage a caseload and keep track of multiple tasks and appointments.

  • Certifications and Licenses (may vary by employer):

    • Some employers may require specific certifications, such as:

      • CPR/First Aid certification.

      • Mental Health First Aid or other specialized certifications in mental health or substance use support.

  • Personal Attributes:

    • Compassionate and non-judgmental attitude.

    • Patience and resilience, especially when dealing with challenging or complex client situations.

    • Adaptability: The ability to handle unexpected changes and work with clients in dynamic and sometimes difficult situations.

  • Ethics & Professionalism:

    • Ability to maintain confidentiality and adhere to legal and ethical standards in all aspects of the job.

    • Professionalism when interacting with clients and other service providers.
